Paper, Order, or Assignment Requirements

• State-adopted student academic content standards and/or Common Core State Standards need to be listed with the number and text of each standard. If only a portion of a standard is being addressed, then only list the part or parts that are relevant.)
• Learning objectives are different from the content standards, be specific. For the Formative Assessment: Develop or adapt a formative assessment that will allow you to assess whole class learning. It should be an assessment that is completed by an entire class of students who would experience the above learning segment. The assessment should reflect the work of individuals, NOT groups, but may be individual work from a group task. The assessment should provide opportunities for students to demonstrate conceptual understanding, computational/procedural fluency, and mathematical reasoning/problem solving skills. Be specific as to how this assessment provides these opportunities.
• Instructional strategies and learning tasks need to be stated in detail, including what you and the students will be doing. What is your role? What are the expectations or outcomes for the students? How will you support diverse student needs. • When listing instructional resources and materials state why you are using these to engage students in learning.
• When laying out the lesson supply questions that you would ask and responses you would expect to get and misconceptions that you need to be alerted to. (the beginning, middle and end of the lesson should be stated)
