Paper, Order, or Assignment Requirements

Your team (the same team as your group project team) are working for a local hospital. The CEO, Waynet Ramos, has asked your team to help her put together some forms and reports in a database. As all members in your team are very busy, your team decides that each member of your team create a form or report independently in the database. Your team will use Group Tools such as Group Discussion Board and File Exchange in your group on the Blackboard for this assignment problem. First student opens the attached file hospital.accdb file, complete the required work, save the file, upload the file to File Exchange on the Blackboard, and then notify the next team member. The remaining students open the file in File Exchange on the Blackboard, complete the required work, save the file in the File Exchange and notify the next team member. The last student completes his/her part, changes the file name to HospitalFinal.accdb, and then save the file in the File Exchange.

Student A: Open the attached file. Create Patients Information Report, which includes fields of First Name, Last Name, Address, City, State, Zip, and Room Number. Name this report as Patients Information–YourName. Save the file and upload the file to File Exchange in your group on the Blackboard. Notify the next team member.

Student B: Open the file in File Exchange. Create Doctors Report, which includes fields of Doctor Name, Patient Last Name, Patient First Name, and Room Number. Name this report as Doctors Information-YourName. Save the file to File Exchange and then notify the next team member.
