For this assignment, you will be creating a case study, which is an account of the symptoms, diagnosis, and treatment of a fictional character that you create. You will address each of the four sections below in full to create a complete case study document. Please submit your work as a .doc, .docx, or PDF file. Your paper should be double-spaced and use a professional 12-point font, such as Times New Roman. At the end of these directions, there is a Word form you can use to help organize your work as well as an example assignment to help you better understand what’s expected.

Your assignment will contain four sections. You can separate each section with a title, but it is not required.

Sections of the Case Study assignment:
1.Presenting Symptoms

In this section, please describe the scene when you, the clinician, first meet this fictional client. What do they look like? What are they doing and saying? What is their age and gender? Are there any specific behaviors, gestures, or phrases that this client is exhibiting? Do they seem to know what day/time/year it is? Are they able to give you basic information about themselves, like their name, age, and address?

2.Client Background

In this section, please describe the client’s background. Where did they grow up? What was their childhood like? What was their educational experience? What is their family structure, and how do they communicate with their family? Have they recently experienced any stress or trauma? What about historically? What do they do for work? Do they have a spouse, friends, pets? Where do they live and what is their living environment like?

3.Information on the Disorder, Including a Diagnosis

Based on the information in section two, you will offer a diagnosis or diagnoses (if the client meets criteria for more than one disorder). Using the DSM-V (available through the FSW library) or your textbook, explain why you decided on this diagnosis, with specific symptoms and criteria included. Please also include at least one other diagnosis that might fit your client, but that you decided against, and explain why. (If you went to an M.D. with the symptoms of a sore throat, the doctor would need to consider various diagnoses before deciding on the one that most closely fit your symptoms. Similarly, if you are a mental health provider working with a client, you need to consider different diagnoses. This is called “differential diagnosis.”)
4.Treatment and Prognosis
